Thank you and welcome for your interest in products and services offered through AFI Benefits. On this site, you'll be able to view and obtain Medicare health insurance information that is available in your resident county. You'll be assigned a personal Benefit Specialist who will assist you with answering questions with the objective of providing guidance and making informed decisions about your preferred health insurance choices.
Use Medicare and You to educate yourself on Medicare Parts A, B, C, D and Medicare Supplement Plans. Frequently asked questions and a Glossary of Terms are also available for your reference and review.
Use Medicare 101 to obtain information about Medicare Advantage Private Fee for Service (PFFS) plans, other Medicare Advantage Plan options, and Medicare Supplement Plan information. Review eligibility and enrollment guidelines to understand about the various carriers offering coverage.
Go to Shopping Cart to learn about the plans (including benefit levels) that are available in your service area. This will also allow you to compare benefits to "Original Medicare" or your current Medicare supplement coverage (Medigap) if you have it. Once you are satisfied with a plan, you may apply by contacting your Benefit Specialist. If you have selected a plan that does not offer Part D benefits, go to Prescription Drug Plan to view Prescription Drug Plans that are available in your area.

We do not offer every plan available in your area. Any information we provide is limited to those plans we do offer in your area. Please contact Medicare.gov or 1-800-MEDICARE to get information on all of your options.
Calls will be recorded for quality assurance.
You may be able to get extra help to pay for your prescription drug premiums and costs. To see if you qualify for getting extra help, call: 1-800-MEDICARE (1-800-633-4227). TTY/TDD users should call 1-877-486-2048, 24 hours a day/7days a week; The Social Security Administration at 1-800-772-1213 between 7 a.m. and 7 p.m., Monday through Friday. TTY/TDD users should call 1-800-325-0778; or your State Medicaid office.
Members should be careful to select HMO or PPO plans with integrated drug benefits if requiring prescription drug coverage. If one of these plans is selected without integrated drug benefits, you will not be eligible for Part D prescription drug coverage on a stand-alone basis.
HMO members must receive all routine care from plan providers, except in emergency or urgent care situations or for out-of-area renal dialysis. If you obtain routine care from out-of-plan providers, neither Medicare nor the carrier will be responsible for the costs.
PPO members can go to doctors, specialists or hospitals in or out-of-network. You may have to pay more for the services you receive outside the network and you must follow special rules in order for the carrier to pay for these services. Otherwise neither the carrier nor Original Medicare will pay for these services. Member responsibility may be greater out-of-network since coinsurance is based on the Medicare allowed amount and not on the potentially lower contracted amount.
For full information on plan benefits, please contact customer service at 1-888-234-6578.
