Road Map for Medicare Eligible Retirees/Spouses

Step 1 – Enroll in Medicare Parts A & B – you may complete this step up to 3 months prior to your 65^th birthday. Your Medicare claim number is required for enrollment. If you have not yet received this, please contact your local social security administration office. 

Please be sure to have your Medicare claim number and both Part A & Part B effective dates available when you are ready to enroll. You may find this information on your Medicare Health Insurance card as illustrated below. You cannot enroll without this information.

Step 2 – Review the Medicare publications that the Centers for Medicare and Medicaid Services (CMS) will be mailing to you.  You may also view additional Medicare publications and information at www.medicare.gov or www.afibenefits.com.

 Step 3 – Once you have reviewed your enclosed plan options, call AFI toll-free at (888) 234-6578 to make an appointment with your benefit specialist.
